To make your first visit to Hendon Chiropractic & Wellness
Center as comfortable as possible, you may want to fill
out your initial paperwork prior to your appointment.

If you plan to file your visit with insurance, be sure to bring
your identification card with you so we can make a
copy and start the filing process.
No printer? Don't worry about it. Come in 15 minutes
before your scheduled appointment and we'll have the forms
ready for you to complete.
